This setting doesn't seem to show items for me in the latest beta... Tried creating a new custom command with a link, label icon and the option selected (Only this option)
then created 2 child items with a link label and icon with no options selected.

Not that i really plan on using the feature, just messing around and noticed it 'broken' classic with 2 columns

Do you have "No icons in second column" skin option checked? In that case, sub-items won't be inserted as buttons/icons.

Maybe they should override this setting? as its pointless otherwise to have them

It is currently not possible because the skins have only one set of metrics per column. It will not look good to share the same size, padding, frames, etc. between the inlined buttons and the text items in the second column.

There are no plans in the immediate future to support this.

alternatively (though it may be complicated)
you could gray out the option for all items under the COLUMN_PADDING item (or over it if the skin makes an option to hide icons on left side, if thats even possible (haven't tried)

I would suggest adding it to the tooltip but It would probably just add unnecessary complexity for users

Starting with Classic Shell 4.0.4, "Insert sub-items as buttons" can be used for items in the second column of the Classic style even if icons in the second column are disabled from the Skin tab.
