I am sure I am missing something simple. I am using version 4.1.0. Win 7 style. I want to remove "sleep" and "Hibernate" from the shutdown dialog. When I right click either it shows delete item as greyed out. Edit Item is not but does not work. What am I doing wrong and how do I remove those items?

Where do you right-click on those items? I don't think they show up anywhere as deletable items in the Win7 style.
You can control what items show up in the shutdown menu with the setting "Shutdown menu items" in the Main Menu tab. Read the tooltip for the setting carefully.

I found the issue to be related to two things. Registry settings and group policy. In group policy I had pasted in the settings from hkcu >software> ivosoft> classicmenu> menuitems7 into user configuration> administrative template> classic shell> classic start menu> menu items for the windows 7 style.

And I had merged the hkcu >software> ivosoft> classicmenu> menuitems7 into hklm > software> ivosoft> classicstartmenu> menuitems7

With either hklm or group policy settings in place the ability to make changes is greyed out. It works the same for the menuitems2 and menu items for menu with two columns when classic with two columns is enabled. However when you right click on the option in Classic Menu settings, it just shows to delete greyed out.

I am not sure which supersedes which in hierarchy. I am guessing that CS group policy overrides CS hklm registry but am not sure.

The order priorities is -> Computer Configuration policies > HKLM settings > User Configuration policies > HKCU settings.
If any of the first 3 are set then the setting is not editable in the Settings dialog.
