windows: 8.1; classic shell version: 4.2.1
Logoff isn't there, however it is enabled.

Download the Classic Shell Utility from here: http://www.classicshell.net/downloads/
Run it and send me the log file.

Looks like the "Show Log Off" setting in the Special Items tab is turned off.
