I've been having issues with the shutdown menu. Sometimes it does not show up and I have not been able to find any reason. Has anyone else seen this or is there a way to set a reg key to permanently enable it for all users? I'm running the latest non-beta version on fully patched server 2012.

Are you directly logged in, or through remote session? If you are logged in remotely, then you need to enable the setting "Show Shutdown during remote sessions" from the Special Items tab. That will only turn it on for the currently logged in user. To enable it for all users permanently,
If you are using 3.6.x, add this to registry:
Windows Registry Editor Version 5.00
[HKEY_LOCAL_MACHINE\Software\IvoSoft\ClassicStartMenu] "RemoteShutdown"=dword:00000001
If you are using 3.9.x betas (and for future versions), add this to registry:
Windows Registry Editor Version 5.00
[HKEY_LOCAL_MACHINE\Software\IvoSoft\ClassicStartMenu\Settings] "RemoteShutdown"=dword:00000001
