I'm using Classic Start Menu 4.0.2 on Windows 8.1. When I used to right-click on the start button, I would see about 10 items, including the Control Panel. Now I only see: Windows Explorer, Open, Open all users, Settings, Help, and Exit. I can't find any option that sets this. Is there a way to get it back to showing more items?

You can get the other menu if you hold down Shift. If you don't want to hold shift, click on the setting "Right click opens the Win+X menu" in the Controls tab.
