Anything in particular you want it to do for the hover/pressed states? maybe an alternate color scheme?

You can download a Start button image by right clicking on it and choosing "Save picture as../Save image as..".

Then you can set the button by open Classic Start Menu Settings and picking "Custom" and then "Pick image...." from the first tab (called Start Menu Style).

You can adjust the Start button image size in Classic Shell if the button is too big or small. Open Classic Start Menu Settings, check "Show all settings" at the top and go to the tab called "Start Button". There are more options there. Under "Custom Button", you can enter a value to resize the button on-the-fly. 0 means Automatic i.e. it will use the width (in pixels) of the image as the size. Try values like 48 to 60 for most buttons. Switch to the "Start Menu style" tab again to preview the size changes live. If the size is fine, finally click the OK button.

Which part are you confused about?
Step 1: hover mouse over the image of 3 buttons aligned vertically
step 2: right click
step 3: find 'save image as' (wording may vary depending on your browser)
step 4: click save

Step 5: right click your start button
step 6: left click 'settings'
step 7: from the start menu style page (the default tab) left click the 'custom' radio button
step 8: click 'pick image...'
step 9: locate the image you saved in step 4 (probably in 'downloads' folder)
step 10: hit 'ok' and you should be done
