I have a number of icons that are showing as blank white squares in CSM. They look fine in NSM. I have figured out that it's related to having my DPI set to 125%. At 100% CSM displays these icons correctly. 150% also has the bug. These are the icons:
  • Nitro Reader 3
  • Quicktime -> PictureViewer
  • Quicktime -> QuickTime Player
  • Steam -> The Walking Dead
This is on Windows 7 x64

Which version of Classic Shell are you using? Are you using the latest 3.9.0 beta from the home page or 3.6.8? If you are on 3.6.8, you can try 3.9.0 first as it gets icons in a different manner.
