the start button has nothing to do with Internet Explorer.
are you sure it is iexplore.exe and not explorer.exe?

what if you start ClassicStartMenu.exe manually? Does it crash afterwards?

No, manual start is ok.

You can try two alternative ways of capturing a crash dump:
1) Use the procdump option -e 1. It will save a dump on any exception, not just fatal ones. There is a chance the dump will capture the real problem
2) Create a new DWORD value named "CrashDump" in HKCU\Software\IvoSoft\ClassicStartMenu\Settings. Set the value to 1. Log off and see if it will create a crash dump. The dumps are saved in the %LOCALAPPDATA% folder and will be named CSM_Crash.dmp

In both cases use version 3.9.0. It will provide more useful crash info.
