All Classic Shell does is add a Start Menu to the taskbar and Explorer plugins. Doesn't affect the desktop in any way. Doesn't the wallpaper applied from Personalization Control Panel persist? You can drag the .bat file you want to the Start button, wait for the menu to open and then drop it in the area where pinned shortcuts are there, or inside "All programs". That is one of the easiest ways to add it. Let me know if you could add it.

That means the Classic Start Menu isn't loading at startup. Just place a shortcut to C:\Program Files\Classic Shell\ClassicStartMenu.exe in your Startup folder. Do you see ClassicStartMenu.exe running in Task Manager after you restart?

For Programs, there is a global setting on the 'Menu Look' tab. Set "Multiple columns in sub-menus" to "Use single column (scroll)".

You can also change main menu and sub menu scrolling speed to 6 to scroll at the same speed Windows classic menu did from 'General Behavior' tab.

There aren't plans to add anything to Classic Shell for it would require introducing another separate component from Start Menu or Explorer add-ins. Maybe a Sidebar launcher gadget could fill the void or some free dock solution. There were many launcher gadgets for Vista/7.

By 'thumbnail view', do you mean 'filmstrip view' or do you mean the flat folder thumbnails used by XP Explorer? MS made folder thumbnail style icons non-customizable in Vista/7/8, so they can't be changed. :( They only show in the 3D/angled style.
