FYI: I was looking back through this thread and noticed that in my 8-25-14 post I gave an incorrect path im describing a solution that worked for me. I have edited the post to show the correct path.

Just a wild stab here, but in trying to reproduce the problem, have any of your attempts been made with a system that does not require a login screen at startup?
