It happened on two servers, and the explorer.exe process was not in a hung state.
Maybe the installation should be restarting explorer.exe? |

Windows Server 2012, upgraded from 4.0.6 to 4.2.6. After the installing 4.2.6 the entire start menu was missing!
Restarted the machine and it was back.
Surprised that a reboot was needed. |
